Pfizer Share Card allows each eligible participant to buy a 30-day supply
of most drugs manufactured by Pfizer for a flat fee of $ 15. Some of the covered
Pfizer drugs are:
Aricept® (donepezil HCl),
Diflucan® (fluconazole),
Glucotrol® / Glucotrol XL® (glipizide)
Lipitor® (atorvastatin calcium)
Neurontin® (gabapentin)
Norvasc® (amlodipine besylate)
Procardia® /Procardia XL® (nifedipine)
Viagra® (sildenafil citrate)
Zithromax® (azithromycin)
Zoloft® (sertraline HCl)
Zyrtec® (cetirizine HCl)
The card is FREE. In order to be eligible you have to be a Medicare beneficiary
without public or private prescription drug coverage and with gross income
of up to $18,000 (less then $28,000 for couples). To enroll simply call 1-800-717-6005
and request a membership kit.
To apply to Pfizer Share Program you will need to fill out an application
and mail it along with copies of your previous year tax return (form 1040)
or your Social Security Benefit Summary Statement (SSA 1099). If you qualify,
your will receive your card in 3 to 4 weeks. After you receive you card, simply
present it along with doctor’s prescription, at any Pfizer Share participating
pharmacy. Your savings are instant—right at the pharmacy counter.
